Due to investment, an opportunity has arisen for TWO forward thinking, proactive and driven Pharmacy Technicians to join Home First, supporting patients following discharge to have their medicines optimised and reach an appropriate level of independence.
You should have excellent communication skills, work well in a team and be able to manage your own workload. In addition to working alongside our Home First Pharmacist, you will have the full support of our friendly and dynamic community pharmacy team.

Main duties of the job
As a key member of the multidisciplinary Home First team:Gather information concerning patients’ current medication/medical historyProvide in-reach to the hospital to support discharges to communityCarry out domiciliary visits to patients where appropriateProvide assessment of patients’ ability to manage their own medicinesProduce MAR charts to support medicine administration by Home First care staffProvide medicine-related advice to patients, families and carersLiaise with other service providers to improve the transfer of care between care settingsParticipate in MDT meetings and participate in service developmentCarry out clinical audit collection.The successful candidate must have access to a car and hold a valid driver’s license for work purposes. Qualifications
Essential
- NVQ level 3 in Pharmaceutical Sciences or equivalent qualification
- Accuracy checking qualification
Registration
Essential
- Current professional registration with the General Pharmaceutical Council (GPhC)
Knowledge and skills
Essential
- Significant post qualification experience in hospital, primary care and/or community services
- Evidence of ability to work independently and proactively with limited supervision.
- Excellent communication skills, both verbal and written
Desirable
- Experience of working in a hospital as a ward-based technician
- Experience of working in community primary care GP teams
- Experience of home visits
- Experience of training healthcare professionals/unqualified staff
